Eligible patients with commercial insurance may pay as little as $0 a month.
To be eligible, your patient must:
- Have commercial insurance and coverage for your treatment
- Be a resident of the US or a US territory
- NOT be a beneficiary of Medicare, Medicaid, VA/DoD (TRICARE) programs, the Indian Health Service, or other federal or state healthcare programs, even if patient pays for prescription in cash or if the patient chooses not to use their insurance coverage

Patients with certain insurance-related delays may be eligible to receive a 15-day treatment supply after 3 days.
Eligible patients will be enrolled in the Quick Start Program through enrollment in (ON)Path.
Patients must be enrolled in (ON)Path to access Quick Start, and appropriate patients may be identified during the automatic benefits verification upon program enrollment.
Patients experiencing additional approval delays may receive 2 more shipments.
If eligible, patients enrolled in Quick Start will be enrolled in the Patient Assistance Program.

Patients with continued insurance-related delays may be eligible to receive up to 2 treatment refills (a 30-day supply).
Patients will automatically be enrolled in the Bridge Program following Quick Start if insurance-related delays persist. Eligible patients will receive assistance via (ON)Path enrollment.

- Meet income-based eligibility criteria

The Patient Assistance Program may be available for patients who are uninsured or underinsured and meet other criteria.
Patients must meet certain medical and financial criteria to qualify. To apply for this program patients must consent to income verification and have confirmation of an on-label diagnosis.
Your patient will be assessed for eligibility in the program once enrolled in (ON)Path.
