To enroll your patient in (ON)Path, either download and fill out the form or complete the form directly online.

Fax Completed Form

Once complete, fax the enrollment form along with a copy of your patient’s insurance card and prescription to 1-855-739-2033. Patients will need to provide verbal consent.

Obtain Patient Consent if Needed

If the patient does not provide verbal consent during enrollment, our Patient Access Advocate will reach out to ensure they provide consent (electronically if they are comfortable).

(ON)Path Patient Access Advocate Welcome Call

Once enrolled, your patient will receive a welcome call from an (ON)Path Patient Access Advocate explaining the next steps.
